Please see below for enrolment form and NZ eligibility criteria. Please allow 2 weeks for us to obtain notes from your old GP.

Unwell?  Feel you need to be seen today? Please let us know

Phone early in the morning if you can. If you have a new illness or are worried you might be seriously unwell we will do our best to help you.
