How to enrol...
If you are eligible for publicly funded healthcare in New Zealand you are welcome to enrol with Te Kauwhata Health Centre. You will enjoy the subsidised rates for General Medical Services and will be actively recalled for screening and proactive preventive healthcare interventions such as cervical smears and cardiovascular checks.
Please complete:
1. An enrolment form
2.Transfer of notes request form
3. Welcome form
Please bring these to reception with photo ID and proof of your eligibility for public funded healthcare in New Zealand (NZ passport, NZ birth Certificate, NZ Visa of Residency, Community Services Card) We don't make these rules and are obliged to comply with them.
Remember to enquire at reception about enrolling for Manage My Health, enabling you to ask for advice, request prescriptions online.
