How to enrol...
If you are eligible for publicly funded healthcare in New Zealand you are welcome to enrol with Te Kauwhata Health Centre. You will enjoy the subsidised rates for General Medical Services and will be actively recalled for screening and proactive preventive healthcare interventions such as cervical smears and cardiovascular checks.
Please complete:
1. An enrolment form
2.Transfer of notes request form
3. Welcome form
Please bring these to reception with photo ID and proof of your eligibility for public funded healthcare in New Zealand (NZ passport, NZ birth Certificate, NZ Visa of Residency, Community Services Card) We don't make these rules and are obliged to comply with them. Please allow 2 weeks for us to obtain notes from your old GP. If you need something more urgently please let our reception team know and we will do our best to help
